Miswak is not a trend. In 1986, the World Health Organization (WHO) officially recommended the Miswak twig for dental care — not because of folklore, but because of measurable clinical effects. Today, an estimated 600 million people worldwide use such a twig daily, most of them in North Africa, the Middle East, and South Asia.

The Miswak (Arabic: مسواك, also called Siwak) is a twig or root of the Salvadora tree (Salvadora persica), which primarily grows in arid regions of Africa, the Arabian Peninsula, and the Indian subcontinent. The bark is peeled off, the exposed fibers are gently chewed — and the natural toothbrush is ready. No plastic production. No dentist's kit needed.

The tree itself is remarkably adaptable: it grows even on barren, salty soils that are unsuitable for crops. In parts of Morocco, Egypt, and throughout the Maghreb, it has been an integral part of local herbal medicine for centuries.

The Active Ingredients: What's in this twig?

Fresh Salvadora persica twigs contain over 60 identified substances — many of which have direct effects on oral health. This is not a coincidence of nature, but the result of millions of years of plant evolution in an environment where bacterial protection is vital for survival.

What makes this combination so remarkable is its synergistic action. BITC, for example, is not only antibacterial but also inhibits the enzyme activity of bacteria responsible for plaque formation. No single synthetic product replicates this multi-pronged attack with just one substance.

What the research specifically proves:

• Anti-plaque effect: Several comparative studies show that Miswak users achieve similar or better plaque control scores than toothbrush users — even without toothpaste.
• Gingivitis reduction: Inflammation parameters in the gums measurably decrease with regular Miswak use — which is attributed to the combined effects of tannins, salvadorine, and benzyl isothiocyanate.
• Antimicrobial: In vitro tests show a strong inhibitory effect on Streptococcus mutans, Porphyromonas gingivalis, and Lactobacillus acidophilus — three of the most important causes of tooth decay and periodontitis.
• Bad breath: The volatile sulfur compounds responsible for bad breath are demonstrably reduced by the antibacterial components of Miswak.

A limitation that must be honestly stated: Most studies have short durations and small sample sizes. Long-term data beyond five years is lacking. The WHO recommendation is therefore based on the overall picture of many consistent short-term studies — not individual proof, but a strong pattern.

Correct Application: How to use Miswak

The most common mistake with the first Miswak: too much pressure, too little patience. The twig is not an abrasive — it works through the gentle, even contact of the exposed fibers with the tooth surface.

1. Preparation: Cut or break off about 1–2 cm from the tip of the twig. Remove the outer bark for about 1 cm with a knife or your fingernails until the soft wood fibers are exposed.
2. Soften fibers: For a fresh twig, it's enough to briefly moisten the exposed fibers with water. For a dried twig, place the end in water or rosewater for 30 minutes.
3. Chew until brushed: Gently chew the softened end until the wood fibers fan out and split like a brush. This step takes 1–2 minutes.
4. Brush: Move the Miswak in horizontal and slightly circular motions over the tooth surfaces. Start at the top outside, move to the inside, then bottom — just like regular brushing. 2–3 minutes are sufficient.
5. Include gums: Gently guide the fibers along the gum line — the tannins act directly on the tissue and reduce inflammation with regular use.
6. Storage: Rinse the twig after use and let it air dry. Cut off the used end after 2–3 applications and prepare a fresh end. A twig lasts 1–2 weeks this way.

Miswak in Morocco: Siwak as an Everyday Ritual

In Morocco, Miswak is called Siwak — and it is found as naturally in the markets of the medinas as ghassoul or argan oil. What is considered a discovery in the Western world is a lived norm here for generations.

In the souks of Marrakech, Fès, and Casablanca, fresh Siwak twigs are often sold alongside other natural remedies — bundled, sorted by thickness and freshness. The local expert, the Attâr (herbalist), knows the difference between a twig that can be used immediately and one that needs to be watered first. This expertise does not exist in bottles — it is passed down orally.

The fact that regions with a high prevalence of Siwak traditionally showed lower rates of tooth decay — despite lower dentist density and less fluoridation — is what first drew the WHO's attention to Miswak in the 1980s. Epidemiological observations, not laboratory hypotheses, were at the beginning of this research history.

• You can recognize freshness by the color: A good twig is light brown to yellowish-beige with intact, smooth bark. Dark spots, mold, or very dry, cracked surfaces are exclusion criteria.
• Scent: Fresh Miswak smells slightly sharp and peppery — that's the BITC you can smell. If the twig smells of nothing or musty, it's too old.
• Diameter: Thinner twigs (5–8 mm diameter) are recommended for beginners as they are easier to handle. Thicker twigs require more chewing but have a higher density of active ingredients.
• Origin: Salvadora persica from North Africa and the Middle East is considered high quality. Look for transparent information about its origin.
• Packaging: The twig should be packaged breathably — no airtight plastic that traps moisture and promotes mold.
